Step 1:

All subcontractors must first prequalify prior to being awarded a project. Please be prepared with the following items to complete the prequalification process:

Current Form W-9
Contractor registration certificate(s)
Experience Modification Rating (EMR) worksheets from your workers’ compensation policy
Three most recent OSHA 300A Summary Logs
Current Surety Bonding Letter (if applicable)
Certificate of Insurances (COI) – please see sample copy for reference
Certificate of Insurances (COI) Suppliers – please see sample copy for reference

Step 2:

Please click on the button below to open the NGC Prequalification Form. When completed, download the form with changes, sign it, and use the “Submit Form” button at the bottom of the form.
