Step 1:

All subcontractors must first prequalify prior to being awarded a project. Please be prepared with the following items to complete the prequalification process:

  • Current Form W-9
  • Contractor registration certificate(s)
  • Experience Modification Rating (EMR) worksheets from your workers’ compensation policy
  • Three most recent OSHA 300A Summary Logs
  • Current Surety Bonding Letter (if applicable)
  • Certificate of Insurances (COI) – please see sample copy for reference
